A community organization that meets people where they are

MLJ Foundation is a mission-driven nonprofit focused on the health, well-being and long-term stability of individuals, youth and families. We work through trusted relationships and practical support — in homes, schools, community centers, parks and neighborhood spaces across the Inland Empire and surrounding underserved communities.
Our approach is in-person first, with virtual support when it is the right fit for the person we are serving. When a need falls outside our nonclinical scope, we coordinate a clear connection to licensed providers and specialized partners, then follow through.

Community-based capabilities

Nonclinical services delivered by Community Health Workers and community staff, organized around what families actually need.

Community Health Worker services
Trusted CHWs provide health education, resource connection and follow-through In-person first, with virtual support when appropriate.
Wellness, nutrition & health education
Practical, nonclinical education on healthy eating, chronic-condition self-management, preventive screening and everyday wellness habits.
Youth & family support
Engagement, mentoring and family-centered support that strengthens stability at home and connection to school and community resources.
Maternal & postpartum navigation
Nonclinical maternal, postpartum and family wellness support — peer encouragement, education and connection to licensed providers.
Coverage & benefits navigation

Housing & Community Supports

Built for accountable, compliant community-based care

Community service delivery comes first. Behind it, structured, technology-supported operations keep documentation, compliance, quality review and follow-through organized — so health plans, managed care plans and payer partners can trust how services are recorded.

Intake
Structured intake captures consent, contact, language and needs.
Eligibility & coverage
Coverage and eligibility information is reviewed and recorded.
Service documentation
Each encounter is documented with time, service type and outcome.
Compliance review
Records are checked against internal documentation standards.
Billing-ready workflow
Documented services are prepared into billing-ready records.
EDI / clearinghouse
Standardized workflows for electronic submission and responses.
Reporting & follow-through
Reconciliation, reporting and closed-loop follow-up on open items.
Technology-supported operations
A unified operating platform supports our workflow, documentation and compliance-review infrastructure, keeping intake, encounter records and follow-up organized in one environment.
Readiness, stated plainly
MLJ Foundation is building infrastructure for eligibility workflows, service documentation, compliance review, billing readiness and reporting for Medi-Cal programs and future CalAIM-aligned services. Nothing here represents a current payer contract, plan approval or guaranteed reimbursement.

Community care built on local relationships

Our delivery model is local by design. Staff and Community Health Workers live in the communities they serve, which is what makes doorstep conversations, school partnerships and neighborhood events possible in the first place. We currently serve the Inland Empire and surrounding underserved communities — an initial service area within a model built to grow across California and beyond.
